Vital Statistics have been delegated to the Finance department, and they are responsible for the issuance of marriage licenses and registration of deaths. They are a liaison with the Office of the Clerk, Office of the Registrar General (ORG), Ministry of Consumer & Commercial Relations and Funeral Practitioners.
Marriage License Applications are available by download here (English / French), or at the Greenstone Administration Office or any Ward Office in Greenstone. The Marriage License Application form must be completed and signed by both parties to the marriage. The form must be submitted in person by the applicant and/or joint applicant with the appropriate identification documentation. A current passport or and original birth certificate PLUS one piece of Photo ID is required for both applicants. Applicants 16 or 17 years of age require parental consent. There are no residency or citizenship requirements. Blood tests or medical certificates are not required in Ontario. If a previous marriage was dissolved or annulled within Canada, the original or court certified copy of the Decree Absolute or Certificate of Divorce must be submitted. A divorce judgement is not acceptable. If a divorce was granted outside of Canada, additional forms must be completed. The ORG is responsible for issuing birth certificates for people born in Ontario. A birth certificate is a "foundation identity document" that is required to apply for other forms of identification, such as social insurance numbers or health cards. The ORG is also responsible for registering deaths and issuing death certificates.
